Meeting language is ambiguous. Participants interrupt each other, revisit earlier positions and use shorthand that only the team understands. Extraction should therefore start as a reviewable draft rather than final truth.
Accept, edit or reject
- Accept when the item accurately reflects the discussion.
- Edit when the substance is right but nuance, ownership or wording needs correction.
- Reject when evidence is weak, the issue was resolved or the item should not enter shared memory.
Confidence guides attention
Low, medium and high confidence are review cues, not quality guarantees. Important decisions should still be checked against evidence even when confidence is high.
Human review is not a decorative approval step. It is the boundary between AI extraction and trusted memory.
